Work schedule may be changed to meet operational needs.LANGUAGE REQUIREMENTS: French essential and spoken EnglishNOTE: Applicants must attach a resume to their application.JOB SUMMARY:Reporting to Primary Health Care, the Nurse Coordinator of Primary Health Care Services Management delivers a person-centred, interdisciplinary and intersectoral collaboration process designed to provide safe and high-quality health services tailored to a population with complex needs and strives to promote effective and efficient resource utilization.The Nurse Coordinator of Services Management supports heavy users of emergency and/or hospitalization services in achieving their personal recovery objectives by helping them build ties to the community, promoting their maximum autonomy, helping them acquire the resources and life and coping skills they need, and providing ongoing support. The Nurse Coordinator of Services Management collaborates with physicians' offices and all other primary care points of service as well as with emergency room and intrahospital staff to identify and manage potential clients by keeping them in the home with health and support services and other individualized support.As part of an interdisciplinary and intersectoral team, the Nurse Coordinator of Services Management helps evaluate, plan, implement and deliver person-centred intervention and service plans in collaboration and in partnership with other stakeholders. REQUIREMENTS: Member of the Nurses Association of New Brunswick (NANB);Bachelor of Science in Nursing;BLS (basic life support) certificate and maintenance of certificate;Minimum of five years' experience in a clinical setting;Minimum of two years' clinical experience in case management or clinical coordination;Minimum two years' clinical experience with a vulnerable and/or complex needs population;Professional experience in primary health care considered an asset;Knowledge of the health services provided in the health networks and the community;Ability to work independently and as part of a team;Physical ability to perform assigned work;Good work history (performance and attendance);Adherence to professional ethics principles, the Network's management philosophy and organizational values;Adherence to Vitalité Health Network's confidentiality rules.
